Open enrollment ends on Friday, October 18 at 11:59 p.m., and elections are effective on November 1, 2019.  Please click the following links to view the Open Enrollment Benefit Guide and the Overview.

Now is your opportunity to make changes to your benefits for next year.  Mid-year benefit changes are not permitted during the year without a Qualifying Life Event. Even if you are not making changes to your plans or coverage levels, we encourage you to review your personal information and life insurance beneficiary information at this time by calling the Benefits Service Center or accessing the online portal. Finally, please retain your Confirmation Statement for your records following open enrollment. If you complete an active enrollment election, you will receive a statement via email if we have an email address on record.

Note: New medical, dental, and vision plans are being offered. Please review the plan information carefully and call the Benefits Service Center if you have any questions.

Important Updates and Plan Information


Medical, Dental, and Vision
Each year we evaluate our medical plan options to provide our employees with quality healthcare coverage at the most competitive cost. The medical, dental, and vision plans will be insured by Anthem effective November 1, 2019.
 
What’s Changing?
 
Medical - Anthem
  • New premium structure: More affordable coverage for employees who wish to insure only a spouse or child(ren)
  • Plan changes including increased deductibles and coinsurance to keep the payroll deductions as low as possible
  • Elimination of the Health Savings Account (HSA) plan option: Employees who do not complete an active enrollment election for Open Enrollment will be enrolled in the Anthem Blue Essential Open Access POS at your existing HSA coverage level.
  • The Anthem plans are open access – no selection of a primary care physician, no specialist referral requirement. Although both plans include out-of-network coverage, the member out-of-pocket cost is substantially higher when you visit out-of-network providers.
  • Changes to prescription drug and participating lab benefits
  • New 4 tier deduction structure - you will be enrolled in the coverage level based on your currently covered dependents.
  • Elimination of Health Savings Account (HSA) option - existing members will be enrolled in Anthem Blue Essential Open Access POS plan.
  • Higher out-of-network employee costs. Access anthem.com and select Blue Open Access POS (Select Network) for provider information.
  • New lab and pharmacy benefits
  • Anthem Blue Essential Open Access POS members will receive Critical Illness and Accident benefits for you and covered dependents.
  • Deductibles and out-of-pocket maximums are per calendar year.
  • Blue Essential Open Access POS does not cover allergy tests, chiropractic care, or therapy (physical, occupational, or speech).
 
Dental and Vision - Anthem
  • New premium structure: More affordable coverage for employees who wish to insure only a spouse or child(ren)
  • Anthem dental and vision plans are similar to current plans in terms of plan design and cost.

 

Other Benefit Updates and Notes

  • No changes to the other benefits or premiums offered by Jim Ellis.
  • Reminder: Life insurance and disability premiums are based on your age. Premiums are based on the appropriate rate for your age as of November 1, 2019.

 

Wellness Reminder
Employee health and wellness continues to be a priority for Jim Ellis.  When you comply with the requirements by the due date, you avoid potential surcharges.

  • Biometric screening: In order to avoid the $25 per month surcharge, you must complete the screening by the due date.
  • Non-tobacco affidavit or active participation in tobacco cessation program for tobacco users: In order to avoid the separate $25 per month surcharge, you must complete the non-tobacco affidavit or be a participant in tobacco cessation by the deadline.

Important Reminders


The plan year is November 1 - October 31.  After you complete your enrollment, you will receive a Confirmation Statement via email. Please review this statement and contact the Benefits Service Center should you have any questions.

  • It is important that your life insurance beneficiary information is accurate!  You will have the opportunity to review and update this information during your enrollment.
  • You have the opportunity to learn more about your benefits and ask questions during your enrollment by calling the Benefits Service Center.
  • Be sure to have the SSNs for all dependents you wish to cover on your benefit plans.

ID Cards


Medical, Dental, and / or Vision Plan Members: One Anthem IDcard is provided for medical, dnetal, and vision coverage (if applicable).  New ID Cards will be issued by Anthem for all plans. Look for your ID cards in the mail to your home address by early November.

Enrollment Instructions


Call the Benefits Service Center at 770-295-1100 or access the Enrollment Portal.

  • Telephonic Enrollment: The Jim Ellis Benefits Service Center is available Monday - Thursday from 8am to 6pm and Friday from 8am - 5pm.  You can speak with a trained Benefits Specialist who will answer your benefits questions and complete your enrollment.  English and Spanish Benefits Specialists are available to assist you.
  • On-line Enrollment: Access the Enrollment Portal.  All passwords have been reset, so you will need to click on the "Reset ID and Password" link to continue.  Enter your personal information as requested and create your case-sensitive password.  Then, you will return to the login screen and enter your last name + date of birth (mmddyyyy) as your User ID, and then your newly created case-sensitive password.
  • You will receive a Confirmation Statement via email following the completion of your enrollment.
