Program Overview

Eligibility

New Hires:

  • Benefits for New Hires will be effective the 1st of the month following 60 days of employment.
     
  • The Employee Assistance Program through SupportLinc is available immediately, on your date of hire.
  • You are required to enroll in benefits by the 25th of the month prior to your effective date.

    • If you do not call to enroll, your voluntary benefits, including medical insurance, will be waived.

    • Annual Open Enrollment is held in November and the plan year begins January 1. Should you wish to change your elections after your effective date, you will need to wait until the next Annual Open Enrollment unless you have a qualifying life event.

Payroll Deductions:

  • Medical, Dental, and Vision premiums are pre-tax (before taxes are withheld).  This lowers the amount of taxes you pay and reduces your net costs for benefits. 
     
  • Life, Disability, Critical Illness, and Accident benefit premiums are deducted on a post-tax basis.

Dependent Child(ren):

  • Eligible child(ren) include:
    • Natural child, adopted child, step child, or child for which the employee has legal guardianship. 
       
  • Dependents are eligible until the end of the month in which they turn age 26 (unless disabled).

Spouses:

  • Only legal spouses are eligible (as defined by Georgia law).
  • Same gender, legal spouses, are eligible.

  • Common law spouses not eligible.

  • Domestic partners not eligible.

  • Health Plan:  Spouses with other employer-sponsored coverage not eligible for medical coverage.

 

For employment separations, the medical plan coverage end date is the date of separation.  In certain cases, medical plan premiums may be paid by employees through the end of the pay period, but the medical plan coverage end date could be prior to the end of the pay period.   

New Hires

Benefit Enrollment

You can enroll in your benefits one of two ways:

  1. Call the Jim Ellis Benefits Service Center at (770) 295-1100.  Service Center hours are Monday – Thursday from 8:00 a.m. to 6:00 p.m. and Friday from 8:00 a.m. to 5:00 p.m.  You can speak with a trained Benefits Specialist who will answer your benefits questions, handle on-line password resets, and complete your enrollment.
  2. Online with Zevo Benefits

    If this is your first time accessing the Zevo Enrollment Portal, you will be prompted to enter your e-mail address, last four digits of your Social Security Number, and date of birth. The system will identify you by these credentials and allow you to create a password. Once you have created your password, you will be able to complete your Enrollment. Note: If you have already registered, simply click Login.

After you have completed your enrollment, you will receive a confirmation statement for your benefit elections.

Please use the Resources page and the individual product pages which contain detailed information about your benefits.  In addition to the information on this website, you may contact the Benefits Service Center via phone at (770) 295-1100 at any time during the year for benefit questions.

Information Needed to Enroll

Please have the following information ready when you enroll:

  • Your name, date of birth, and Social Security Number.
  • The name(s), date(s) of birth, and Social Security Number(s) of your dependent children up to age 26. (Required!)  Dependent children include your natural children, adopted children, step-children, and children whom you claim as dependents for Federal Income Tax purposes, or for whom you have legal guardianship.
  • The name, date of birth and Social Security Number of your spouse, if applicable. (Required!)
  • Your current address, so that we can update our records. This will also ensure that your ID cards and other important benefit information are sent to the correct address.
  • The full name and relationship of your life insurance beneficiary (your beneficiary must be at least 18 years old).

 

The collection of dependent Social Security numbers is a legislative requirement for enrollment in the health, dental, and vision plans.

Qualifying Life Events

No enrollment changes are allowed to your benefits during the plan year, except in the case of a Qualifying Life Event. 

Qualifying life events that can result in changes to your benefit coverage include the following:

  • Marriage or Divorce
  • Birth or adoption of a child
  • Loss of a dependent
  • Medicare entitlement
  • Change in spouse's employment that affects benefits
  • Loss of other group coverage

 

Qualifying Life Events:

If you have a qualifying life event, notify the Benefits Service Center of the event and your new elections.  You must also provide the necessary supporting documentation to the Benefits Service Center via fax at 866-597-2202 or via email at benefits@jimellisbenefits.com within 31 days of the change. If you do not do so within 31 days, you must wait until the next open enrollment period to make any benefit plan changes.

Employee Portal Information

The Jim Ellis Employee Portal is your online resource for:

  • Events, news, and announcements within the company (Located on the Welcome page)
  • Viewing and updating your personal information, such as getting married or moving
  • Viewing attendance, paid time off, or sick time
  • Viewing and printing benefits forms (located on the Resources tab)
  • Searching the Company Directory
  • Accessing your jimellis.com email
  • And much more..

You can access the Portal from anywhere at anytime, using a laptop, desktop or mobile device.  If you do not have your own desktop, you can access your jimellis.com email through the Portal. A link to the email login screen is located on the Welcome Page. You are encouraged to check your email regularly for inter-company communication.
